Information you send through this public website — such as the question form — is not a medical record. It is voluntary, it is used to answer you, and it does not create a patient-provider relationship. Your medical records live in the clinic's HIPAA-covered systems (Tebra), not on this website.

If you are a patient of Dr. Zuleikha Tyebjee, MD, your protected health information (PHI) is handled in accordance with HIPAA requirements through our clinical systems, which are separate from this website.

We treat all health-related messages with the highest level of confidentiality and do not sell, share, or disclose them to any third party for marketing or advertising purposes.

  • GoHighLevel (GHL): Our marketing and communications platform. Your name, email, phone number, and consent choices may be stored in GHL.
  • Tebra: Our scheduling and clinical-intake platform. Medical information belongs in Tebra's clinical systems, not in public marketing forms.
  • Service providers who assist in operating our website and services, under confidentiality agreements.

7. Your Rights & Data Deletion

  • Request access to the personal information we hold about you.
  • Request correction or deletion of your personal information.
  • Opt out of email communications by clicking "unsubscribe" in any email.
  • Opt out of SMS communications by replying STOP.

Clinical-record requests are handled through the clinical system and applicable law.
