These Terms of Service ("Terms") govern your use of the Mindful Medical Hormone Replacement Therapy website and services operated by MMWL PLLC and Dr. Zuleikha Tyebjee, MD ("we," "us," or "our"). By accessing or using our website, you agree to be bound by these Terms.

1. Use of Services

Our website provides educational content about menopause, hormone, and metabolic care, and information for scheduling a telehealth visit with Dr. Zuleikha Tyebjee, MD. You agree to use our services only for lawful purposes and in accordance with these Terms.

You must be at least 18 years of age to use our services. By using our website, you represent that you are 18 or older.

The content on this website — including articles, condition guides, and question lists — is for educational and informational purposes only. It does not constitute medical advice, diagnosis, or treatment.

3. Scheduling and Payment

Visits are scheduled through our booking partner, Tebra. HRT visits are $250; separate optional medical weight-care visits are $150. Visits are billed after they take place. Medication and labs are separate. There is no required membership, subscription, package, or enrollment.

4. Communications Consent

By providing your email address or phone number on our website, you consent to receive communications from us, including:

  • Educational emails related to our care and services
  • Follow-up communications regarding a question you sent or a visit you asked about
  • SMS/text messages if you have provided your phone number and opted in

You may opt out of email communications at any time by clicking "unsubscribe." You may opt out of SMS by replying STOP.

All content on this website — including our care approach, educational material, copy, and design — is the intellectual property of MMWL PLLC and Dr. Zuleikha Tyebjee, MD. You may not reproduce, distribute, or create derivative works without our express written permission.

To the fullest extent permitted by law, MMWL PLLC and Dr. Zuleikha Tyebjee, MD shall not be liable for any indirect, incidental, special, or consequential damages arising from your use of our website or services. Our total liability for any claim shall not exceed the amount you paid us in the 12 months preceding the claim.
